The address question type allows you to capture address details from a respondent.
Options
Note that you can change the text shown under each input field. For example, you may change state to province or county. To change “State” to “Province,” simply click on Edit on the address question type. In the textbox above “State,” type in “Province” and click the Done button.
Include a note – Checking this will allow you to enter a note that will appear under your question.
Question is mandatory – Checking this will make the question mandatory, meaning that the user must enter an address before the survey can be completed.
Place – Adding Place includes an Address Line 1 and Address Line 2 in your question.
City – Includes a text box in which participants can enter their city.
State – Includes a text box in which participants can enter their state.
Zip Code – Checking this box includes a field to allow a respondent to enter a zip code.
Note, that you can change “Zip Code” to “Post Code” or “Postal Code” if you like. Simply click on Edit on the address question type. In the textbox above “Zip Code,” type in “Postal Code” and click the Done button.
Country – Checking this box includes a dropdown in the question with a list of countries that the respondent can choose from.
Note that you can add media to the address question.
